Welcome to South East London CCG Hub.

Xyla Elective Care is a nationwide CQC registered cardiology diagnostics services provider, works collaboratively with South East London CCG to deliver community cardiology diagnostic services to Lambeth’s local population.

These services are being delivered at Cardiovascular Diagnostic Centres (CVDCs) across the borough and consist of the following diagnostic tests: 12 Lead ECG recording and interpretation, Ambulatory ECG Monitoring, Ambulatory Blood Pressure Monitoring (ABPM) & Home Blood Pressure Monitoring (HBPM).

Referral Pathway

There are 3 interim Cardiovascular Diagnostic Centres to undertake this service.

The services will be reviewed and assessed in the first 6 months to determine what if any service improvements are required, as well as whether they meet the needs of referrers and the patient population.

Your CCG have ratified a specific referral pathway regarding which CVDC each GP Practice should refer their patients into for diagnostic tests.

EMIS Referral Pathway

An EMIS referral pathway allows for a swift, seamless, and safe process. Your practice can immediately access available appointments at your appointed CVDC and book your patients for an appointment. The CVDC then views these appointments and has automatic access to your completed clinical referral form, they then in turn return the complete report within EMIS where a task is raised for your attention.

Service Timelines for Referrers

There are clear referral to report times, which form part of this service that we are all required to adhere to. Referrers are to ensure that once they refer a patient for a diagnostic test that they are booked in for their appointment within a specific timeline. CVDCs are to ensure that they return completed reports to you within a specific timeline and Xyla Diagnostics are to ensure that they turn around reports back to the CVDC within those timelines so that the CVDCs meet these.

Clinical Pathway

The service has been set up with robust clinical pathways to ensure patient safety and that clinical standards are met.

The is a clearly defined cohort of patients who can be referred into this service, Inclusion Criteria, as there is a clearly defined cohort of patients that should not be referred into this service, Exclusion Criteria.

Quality Pathway

Maintaining quality standards are important to ensure best practice is achieved.

To support you the referrer, the ECG Report Template includes guidance to support you with your clinical decision making. In this section you have access to view, Example of the GP management report.

A RAG Algorithms Table is utilised to support clinical findings, and this can also be found here. Additionally, there are other quality processes which are stored in this section, to provide you with reassurance that the clinical service is governed from a quality perspective.
